Hounslow Community Psychosis Rehab Team: Band 6 Care Coordinator

West London NHS Trust's Community Rehabilitation Services has been operational for approx. 3 years and has recently gone through a period of review. We are the process of implementing the findings of the review and so there will be a shift from the Care Coordinator role towards named worker which will allow for more specific professional clinical input to occur. The emphasis of the changes is to ensure that the service meets and exceeds the current quality standards for community rehab for psychosis and aims to provide the highest standards of clinical care possible. Main duties of the job
These are exciting times for community rehabilitation services and we would welcome applications from those who would like to be part of this journey. What we are looking for are highly motivated and enthusiastic clinicians with a passion for psychosocial rehabilitation who are excited by service development. The successful applicant will be supported by senior and professional management, as well as working within a highly engaged team who are keen to continue to make positive changes. Previous experience of rehabilitation is less important than a willingness to learn and commitment, drive and creativity in meeting the challenges for the team as we move into the future.

Current professional qualification in Mental Health Nursing/Social Worker/OT.
Registration must be live with a professional body such as NMC or other relevant body
Evidence of post registration training and CPD
