Adults Social Worker – Integrated Network Team Bridgend Agency | Initial 3 month contract Full time preferred Up to £35.32 per hour Hybrid working – 2 days home / 3 days on site Bridgend Council is looking for an experienced Adults Social Worker to join one of their Integrated Network Teams on an initial 3 month contract. Full-time candidate required but happy to receive applications from those considering part time. This role is based in Pyle. You will be working as part of a well-established multi-disciplinary team supporting adults aged 18 with frailty, long-term conditions, disabilities and often additional mental health or emotional wellbeing needs. The role: Adults community social work within an Integrated Network Team Managing complex cases with a strengths-based, person-centred approach Care coordination and multi-agency working Regular visits required (driver essential) Hybrid working - 2 days from home, 3 days on site The team includes: Social Workers, District Nurses, OTs, Physiotherapists, CPNs, SALTs, Dieticians, Pharmacy Technicians and Support Workers – with close links to GPs, Primary Care, clusters and the Third Sector.
