Overview
Community Twilight Nurse - Surrey Downs H&C role at Surrey Downs Health & Care Partnership. To work within the team, contributing to team management and the development of innovative practice. To be responsible for the assessment of care needs and the development, implementation and evaluation of holistic care packages.
Responsibilities
* Work as part of the team undertaking lead responsibilities as negotiated and agreed with District Nurse
* Treat all patients as individuals, respecting their privacy and dignity at all times
* Assess individual care needs, implement and evaluate holistic programmes of care
* Involve, support, inform and educate family/carers
* Promote health and wellbeing of the patient
* Develop effective communication and liaison systems
* Maintain clear, comprehensive, signed and contemporaneous records according to SDHC procedures
* Facilitate/undertake training and professional updating for team and students
* Act as an assessor and mentor to qualified and unqualified staff
* Maintain, develop and monitor standards of evidence-based nursing care
* Maintain professional and clinical integrity in line with NMC guidelines
* Support/mentor team members and participate in clinical supervision activities
* Work without direct supervision and liaise with the senior team member; deputise as required
* Be responsible for timely patient care and data entry/record keeping
* Work in co-operation and partnership with other professionals and agencies involved in the care of the patient
Requirements and Qualifications
* Registered nurse (NMC) with current practising registration
* Experience in community nursing and twilight shifts preferred
* Ability to lead initiatives in collaboration with the District Nurse
* Demonstrated commitment to patient privacy, dignity and holistic care
* Strong communication, documentation and mentorship skills
* Ability to work across different SDHC localities as required
* Engagement with Clinical Governance and Risk Management strategies

Additional details
We are looking for maternity leave cover currently 15 hrs available until June 2026 and an additional 5 hrs available until March 2026.
