What you will do:
* Maintain a Complex Caseload: Assess, support plan, and review complex individual needs, initiating and coordinating outcomes to promote independence.
* Conduct Assessments: Use various health and social care tools, contributing to continuing healthcare assessments.
* Identify Reablement Opportunities: Provide access to reablement services as needed.
* Promote Strength-Based Approach: Embed the ethos of promoting independence and wellbeing, enabling individuals to meet their short- and long-term goals.
* Identify Community Resources: Maximize individuals’ assets using preventative services and funding sources.
* Monitor and Review Services: Ensure coordinated service provision with a focus on wellbeing and outcomes.
* Liaise with Providers: Negotiate with local providers to deliver better outcomes.
* Provide Professional Support: Offer detailed information, advice, and support to individuals and their careers.
* Safeguarding Work: Undertake safeguarding duties as a Safeguarding Officer.
