Social Prescribing Link Worker
Fixed-term: 12 months
Base: Liverpool First PCN (Dr Jude's Practice Riverside, Hornspit Medical Centre and Walton Village Medical Centre – subject to change)
Annual Leave: 28 days + bank holidays FTE (pro-rata for part-time)
Sick Leave: 10 days FTE (pro-rata for part-time)
Liverpool First Primary Care Network (PCN) is looking to recruit one Social Prescribing Link Worker to join our growing multidisciplinary team.
About Us
Liverpool First PCN brings together 10 GP practices serving around 50,000 patients. Our diverse workforce includes GPs, nurses, clinical pharmacists, pharmacy technicians, care coordinators and link workers. We work collaboratively to deliver person-centred care and are now expanding our social prescribing service.
The Role
As a Social Prescribing Link Worker, you will:
* Support patients referred by GPs and other health professionals.
* Help people access community services and activities to improve health and wellbeing.
* Work with patients to address social issues such as isolation, housing, employment and financial concerns.
* Build strong links between practices and the voluntary/community sector.
You will work closely with the PCN Manager (Mr Daniel Westhead) and the Social Prescribing Lead (Dr Anurag Gupta), alongside the wider clinical and non-clinical team.

* Experience in health, social care or community roles.
* Excellent communication and organisational skills.
* A person-centred, empowering approach to supporting patients.
