Offer description

The post holder will:

1. Provide clinical assessment and direct the management of patients.
2. Be autonomous in decision making, including judgments involving highly complex facts and situations that require analysis, interpretation, and comparison of options related to patient care and management on a day-to-day basis within the clinical environment.
3. Identify the need for change, lead innovation, and manage change, including service development, developing a case for change, and negotiating and influencing change.
4. Carry out complex patient clinical reviews, and feedback complex clinical information relating to physical and mental health to enable decisions in care management to be agreed upon and/or changed, which may involve differences of opinion.
5. Lead and chair ward reviews, including initiating and titrating treatment plans.
6. Prepare medical reports.
7. Support doctors' development by arranging opportunities in other clinical areas for them.
8. Review formal patients and collaborate with Responsible Clinicians (RCs) regarding ongoing detention and least restrictive principles.
9. Provide clinical assessment using advanced knowledge and skills to diagnose both differentiated and undifferentiated patients, and develop management plans for optimal care.
10. Provide Advanced Clinical Practitioner Strategic Service clinical leadership.
11. Be responsible for policy and service development.

The post holder must hold an independent prescribing qualification and have advanced specialist knowledge of Mental Health patient pathways.

This Band 8b post is open to applicants with Approved Clinician Status or as a development opportunity for qualified Advanced Practitioners willing to achieve Approved Clinician Status within an agreed timeframe. The successful candidate will be appointed under the All Wales Annex 21 rules.

Hywel Dda University Health Board provides NHS healthcare services across Carmarthenshire, Ceredigion, Pembrokeshire, and neighboring counties, employing over 13,000 staff across various services including primary, community, hospital, mental health, and learning disabilities. Services are provided in:

* Four main hospitals: Bronglais Hospital (Aberystwyth), Glangwili Hospital (Carmarthen), Prince Philip Hospital (Llanelli), Withybush Hospital (Haverfordwest).
* Five community hospitals: Amman Valley, Llandovery (Carmarthenshire), Tregaron (Ceredigion), and Tenby, South Pembrokeshire hospitals.
* Two integrated care centres: Aberaeron and Cardigan (Ceredigion), along with other community settings.
* Numerous general practices, dental practices, pharmacies, ophthalmic practices, and other community services.

This role includes a development plan with staged payments during the first four years, culminating in a full Band 8b salary upon successful qualification and competency achievement. Support and supervision are provided, and failure to meet the requirements may lead to redeployment.
