The 'Virtual Pharmacy Homecare Service' has an exciting opportunity to welcome a Band 7 Pharmacist into the team. The UK Pharmacy Homecare sector is growing rapidly, and the CUH pharmacy homecare team currently manages the supply of medicines directly to the homes of over 9,500 patients. We are looking for an organized, forward-thinking, and motivated Pharmacist to join our dynamic team. This is an exciting opportunity to work with a highly specialized patient cohort and various clinical teams, including Rheumatology, Dermatology, Gastroenterology, and Neurology. You will play a key role in the provision of specialist services: high-cost drugs, medicines optimization, and the management of clinical queries to help us deliver and improve the quality of patient treatment and care, maintaining the safe supply of medicines and treatment to patients' homes.
Responsibilities:
1. To work as a Specialist Pharmacist in the Homecare team, providing services to various clinical areas as part of the overall pharmacy service.
2. To pharmaceutically verify/clinically screen homecare prescriptions.
3. To deputise for the Lead Pharmacy Homecare Specialist.
4. To liaise with senior pharmacy, medical, and nursing staff, other allied health professionals (AHPs), and managers to promote the safe, evidence-based, and cost-effective use of homecare medicines.
5. To participate in and lead, where required, on novel pharmacy projects related to homecare medicines.
