Job Overview
We are seeking a motivated and experienced Registered Nurse (Transformation Facilitator – Band 6) to support the delivery and development of high‑quality community health services within NELFT. This role plays a key part in supporting frontline community nursing teams, enhancing clinical practice, and contributing to ongoing service transformation across our locality.
Working as part of the Integrated Community Team, the post holder will combine a reduced clinical caseload with a strong focus on practice education, supervision, leadership, and quality improvement. The role supports staff to develop competence, confidence, and consistency in practice, ensuring safe, effective, and person‑centred care in line with Trust values and national standards.
Main duties of the job
* Provide clinical leadership and support to community nursing teams within a specified locality
* Hold a reduced clinical caseload, ensuring high standards of assessment, care planning, and evaluation
* Support staff development through competency‑based supervision, training, and education
* Facilitate learning in practice for newly appointed staff, apprentices, students, and integrated care colleagues
* Contribute to service transformation, quality improvement, and innovation initiatives
* Liaise with GPs, partner organisations, and wider multidisciplinary teams to support integrated care delivery
* Deputise for the Team Lead when required and support day‑to‑day team management
* Ensure compliance with Trust policies, governance requirements, and professional standards
Detailed Job Description And Main Responsibilities
The post holder will work autonomously within the Integrated Community Team, providing expert clinical advice, leadership, and support to nursing staff across a defined geographical area. Approximately half of the role focuses on clinical practice, with the remainder dedicated to education, transformation, and workforce development activity.
Key responsibilities include managing and prioritising workloads, delivering competency‑based supervision, supporting appraisal and performance development processes, and identifying learning and development needs within the team. The role includes supervising students, supporting newly qualified staff, and acting as a mentor and role model for high‑quality, evidence‑based practice.
The Transformation Facilitator will actively contribute to service improvement through audit, evaluation, and participation in transformation projects, working closely with the Clinical Leads and other senior colleagues. Effective communication, partnership working, and risk management are essential components of the role, alongside a strong commitment to patient safety, clinical governance, and continuous improvement.
The role requires flexibility, the ability to work across multiple sites, and confidence in managing complex clinical, professional, and organisational situations.

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period unless currently partway through a probationary period or employed as bank staff.
High Cost Area Supplement (Fringe Zone)
This post attracts a High Cost Area Supplement (Fringe Zone), which covers areas just outside the M25, including parts of Surrey, Hertfordshire, Essex, and Kent. The Fringe zone weighting is 5% of the basic salary, with a minimum payment of £1,346 and a maximum of £2,270 per annum, pro rata for part‑time staff.
