Overview
The Intensive Home Treatment Team is a citywide service for older people experiencing an acute episode of a wide range of mental health difficulties and younger people with a diagnosis of Dementia who are in crisis. It is based at The Mount. IHTT is made up of Nurses, Occupational Therapists, Psychologists, Social Worker, Health Support Workers and Doctors. The team provide robust alternatives to admission; facilitate early transfer from hospital, as well as preventing the breakdown of a caring situation both at home or in a residential or nursing care setting when the Care Homes Service is unable to attend. Service operating hours, 8am-9pm, 7 days per week. Essential car driver and access to a vehicle.
Responsibilities
* You will work as part of a multidisciplinary team providing assessment, care planning and care delivery.
* Provide highly skilled care, psychological interventions, clinical assessment, and role-specific assessments.
* Develop plans of care and safety plans, conduct risk assessments and carry out home-based treatment interventions.
* Carry out assessments/risk management and promote recovery and inclusion.
* Work autonomously and manage a team caseload.
* Operate across a 7-day week with extended hours (12-hour shifts) including weekends and Bank Holidays.
