It is with great pleasure that we invite applications for our Mental Health Advanced Nurse Practitioner post. As the post holder, you will work within the Perth & Kinross Adult Community Mental Health Service as part of the redesign of care for adults with a wide range of mental illness.
The role offers an excellent opportunity for Registered Mental Health Nurses to improve outcomes and the therapeutic experience for individuals with mental illness within our community settings.
The post holder will pro-actively manage and review individuals who are experiencing acute and complex mental ill health and, in situations of deterioration, assess and provide a differential diagnosis and develop a treatment plan, working in close collaboration with the wider CMHT and General Practice. Working closely with people and their families/ carers, the post holder will identify clinical issues quickly and aim to prevent avoidable deterioration and admission/ readmission to inpatient care. The post holder will also interface with in-patient services to facilitate timely discharge and efficient and person centred transitions of care.
The post holder will play a key role in the leadership and development of advanced nursing practice within the Mental Health services across Perth and Kinross, will participate in the delivery of specialist education and training programmes, and will work collaboratively with health and social care colleagues across the Partnership to deliver high quality evidence based and person centred care to individuals with mental illness.
As the successful candidate, you will be an experienced mental health nurse and as a minimum hold a post graduate diploma in Advanced Practice including Non Medical Prescribing at Masters level.
This role is an excellent opportunity for an individual with highly developed communication skills and experience of working effectively across professional boundaries. The ability to travel across Perth & Kinross is essential. Please see Job description for other requirements of this post.
