Responsibilities
* Lead services in the Frailty and Care of the Older Person department
* Work closely with the multidisciplinary team to promote clinical and cost‑effective prescribing through evidence‑based guidelines
* Develop, audit and monitor Medicines Management processes within the service
* Provide training and supervision to departmental staff
* Promote and embed effective medicines management systems into patient care
Qualifications
* Previous experience working within a clinical pharmacy environment
* Completed a post‑graduate diploma in clinical pharmacy
* Evidence of multidisciplinary teamwork is advantageous
* Prescribing qualification or be prepared to work towards one
* Commitment to continuing professional development
