Outreach Family Support Worker
Liverpool Early Help
Liverpool Early Help utilises a strength-based approach to provide empowering interventions to children and families, helping to address emerging worries and promote positive change. Our aim is to improve long-term outcomes for children and young people.
As an Outreach Family Support Worker, you will act as the lead professional, coordinating multi-agency support to ensure a whole-family approach in addressing identified needs.
Key Responsibilities:
Undertake Early Help assessments with families
Create and review support plans
Chair Team Around the Family (TAF) meetings
Engage in direct work with children and young people to explore their views and worries
Monitor outcomes and adapt interventions to ensure progress is achieved
Essential Qualifications and Experience:
NVQ Level 3 in Childcare and Education, NND, or Health and Social Care (or equivalent)
Experience of direct work with children, young people, and their families
Proven experience of effective collaborative and multi-agency working to support children and families
Key Skills and Abilities:
Ability to maintain accurate and reflective records in line with Recording with Care guidelines
Strong communication and interpersonal skills to build relationships with families and professionals
Ability to work both independently and as part of a team across a range of services including Health, Education, Children’s Centres, and Schools’ Family Support Services
Commitment to promoting positive outcomes for children and young people through early intervention and support
