Are you an enthusiastic physiotherapist looking to develop your skills within a supportive community therapy team? We have an exciting opportunity for a Band 5 physiotherapist to join our service, delivering high‑quality, person‑centred care to patients in their own homes and community settings.
As a Band 5 physiotherapist, you will work as part of a multidisciplinary team to provide comprehensive rehabilitation and therapy interventions, supporting individuals to achieve their goals and maintain independence. This includes working alongside other therapy colleagues to deliver coordinated, high‑quality rehabilitation. You will manage a varied caseload, including complex cases, and contribute to service development and quality improvement initiatives.
You’ll be joining a friendly, supportive team known for its collaborative spirit and commitment to patient‑centred care. Our teams are embedded within their local communities, making a real difference to the lives of local people.
Applicants must be HCPC registered physiotherapists with full access to a vehicle and a full driving licence. The role is open only to applicants from the Hampshire & Isle of Wight NHS system.
Responsibilities
* Work with the multi‑professional Locality Health Team to deliver rehabilitation programmes within the community, collaborating with community and inpatient teams, social care, primary and secondary care.
* Manage a caseload under supervision as required, responsible for a defined area of service, including comprehensive assessment, planning and implementation of treatment programmes for patients requiring intensive support or rehabilitation, guided by more senior clinicians when needed.
* Participate in and maintain your own clinical professional development (CPD), keeping abreast of new developments and incorporating them into practice under a supervisor’s guidance, and providing supervision and assessment to rehabilitation assistants and students on placement.
* Work collaboratively with partners in health and social care to establish a whole‑systems approach to managing patients with complex care needs, maintaining a community pull for patients being admitted to secondary care.
Qualifications
* Degree or Diploma in Physiotherapy, state registered with the HCPC.
* Current UK professional registration.

Benefits and Working Details
Band 5 – Pay scheme: £32,073 to £39,043 per year (full‑time hours). Contract: Permanent. Working pattern: Full‑time. Job location: New Kings Court, Tollgate, Chandlers Ford, Hampshire, SO53 3LG, United Kingdom.
