Highly Specialist Cardiac Pharmacist (Supporting Primary Care)

(37.5 hours a week including back-up on call and weekend commitments

An exciting opportunity has arisen for a motivated and enthusiastic pharmacist to join the cardiac team at Barts Heart Centre within Barts Health NHS Trust.

The post holder will work alongside the cardiovascular prevention primary care pharmacy team and deliver quality care to patients through efficient medicines optimisation. The post provides several opportunities for professional and personal development, contributing to quality improvement projects and supporting the delivery of world‑class and innovative cardiovascular services


Main duties of the job

There is a need for close coordination with the cardiovascular prevention primary care pharmacy team and wider primary care MDT such as general practitioners, pharmacy professionals, nurses, practice managers, community pharmacy, etc to plan workload and priorities. This involves optimising cardiovascular medicine, for long term conditions related to cardiac disease across primary care services within North‑East London (NEL) and beyond.

The post holder will work with primary care teams to continually develop the service, while maintaining high standards and achieving best practice to meet the needs of patients and the Trust.

The post holder communicates effectively with patients to help optimise their pharmaceutical management and through liaising with other health care professionals to design, implement plans to optimise health outcomes.

Person Specification


Experience

* Significant post registration hospital and or primary care experience in cardiovascular medicine
* Experience working as part of a multi‑disciplinary team to provide patient care
* Ability to undertake and facilitate quality improvement, reflective practice, teaching and CPD
* Experience of providing hospital‑based clinical service within cardiac services
* Operational management experience in a complex environment
* Experience of managing staff
* Experience of using GP clinical systems such as EMIS, SystemOne and Vision


Skills

* Excellent interpersonal skills
* Able to complete documentation accurately with attention to detail
* Able to accurately follow written procedures under pressure
* Ability to plan, prioritise and organise self, and others
* IT literate; able to use Microsoft office with intermediate IT skills including, accurate data entry and use of specialist computer programmes
* Excellent verbal and written communication and presentation skills suitable for a range of audiences, including chairing of meetings.
* Ability to work autonomously and supporting junior members to deliver service provision.
* Experience in local implementation of national standards and guidelines
* Demonstrated ability to negotiate with and influence senior medical staff and senior management
* Demonstrates ability to deliver staff training


Knowledge

* Sound knowledge of drugs and therapeutics in cardiology at Diploma level or equivalent
* Thorough understanding of clinical governance and clinical risk management
* Knowledge and awareness of current national standards including recent cardiovascular aspects of NHS long term plan.
* Understanding of the importance of adherence to SOPs and legislation
* Demonstrates a good understanding of patient safety initiatives
* Knowledge of the law and relevant guidance relating to medicines, pharmacy, rationing decisions in the NHS
* Knowledge of the medication issues at the interface of primary and secondary care.
* Awareness of national datasets such as CVDPREVENT


Qualifications

* MPharm or equivalent
* Registered as a pharmacist with the GPhC
* Postgraduate Pharmacy Qualification or equivalent experience (minimum postgraduate diploma level)
* MSc in Clinical Pharmacy or equivalent.
