Position Summary
As a Community Health Care Assistant working in the Out of Hours District Nursing Service you will be integral to the care of the patients.
The Trust is focused on supporting the Community and helping patients remain independent in their own home. You will actively assist the Community Nursing Team in the provision of nursing care and support to patients and families in the community setting, including supporting end of life patients.

Key Responsibilities
• Work alongside a member of the team, working in pairs for safety with full support of your Team Leader and other members of the team.
• Your caseload will be varied to your skills and training requirements. It will have a variety of patients with health and social care needs, ranging from simple and complex patient care to looking after people at the end of their lives who wish to remain at home.
• Work closely with the MDT (Social Workers, GP’s and Allied Health Professionals) to ensure that patient care is appropriately coordinated.
Work Hours & Shift Pattern
The Evening and Overnight district nursing team supports patients from 19:00-08:00 across a 7 day week. The core hours of this post are 3 evenings per week 19:00-00:00 across 7 days covering weekends and bank holidays.
Salary & Sponsorship
Changes to the UK immigration policy introduced on the 9th April 2025 increased the minimum salary threshold for certificates of sponsorship applications for Health and Care Worker Visa from £23,200 (£11.90 per hour) to £25,000 (£12.82 per hour). As per Agenda for Change, if you are new to the NHS your starting salary would be £24,937 (minimum Band 3 salary point from 1st April 2025 in line with National Living Wage requirements). This salary point is below the new minimum threshold, and therefore not eligible for sponsorship.
Unfortunately, this role does not meet the criteria required to qualify for sponsorship.
