Job Overview
Essex Partnership University Trust (EPUT) is recruiting Health Care Assistants (HCA) for its Community Rehabilitation Service within the Collaborative Care Team. This team works with Frailty Virtual Ward, Community Stroke Team, and Early Support Discharge Service to provide rapid assessment and rehabilitation for patients in their homes.
Main duties
The HCA will support person‑centred care and rehabilitation programmes across South East Essex, including Castlepoint, Rochford and Southend. Key responsibilities include:
* Assisting with personal care and daily living activities for patients recovering from conditions such as stroke, orthopaedics, fractured injuries and neurological issues.
* Supporting the facilitation of discharge and preventing hospital admission by implementing rehabilitation programmes.
* Traveling efficiently across the service area to attend patient homes.
* Working in a multi‑disciplinary team over seven days a week, across a range of shift patterns.
* Demonstrating good interpersonal skills when communicating with patients, families, carers and colleagues.

Requirements
Applicants must:
* Be able to travel across South East Essex.
* Be motivated, willing to learn, enthusiastic and possess good interpersonal skills.
* Provide professional references with business contact information covering the last three years of employment.
* Accept that DBS checks will be incurred and deducted from the first salary.
* Understand COVID‑19 vaccination requirements for people directly employed to work in care homes or deployed to a CQC‑registered care home.
* Note that sponsorship is not available for this part‑time role.
