The Colorectal and Stoma Team are looking for a highly motivated and dynamic individual to join our team.
The post holder will work closely with the multidisciplinary team to ensure coordination of specialist nursing care for patients with colorectal cancer through diagnosis, treatment and in follow up care. This is a dual role which also encompasses stoma care. The post holder will offer support and guidance to both newly established and longstanding patients with a stoma.
This is 12 month fixed term position based at High Wycombe Hospital, although cross site working will be required.
Main duties of the job
The main duties of this role include providing a specialist nursing service to patients of all ages with colorectal diseases and those undergoing stoma formation within Buckinghamshire NHS Trust. The Colorectal and Stoma Nurse Specialist is a member of a multi-professional team providing a service to patients with colorectal cancer and other diseases of the colon that may result in stoma formation. They provide support to patients and their carer’s, from the time of diagnosis and throughout the course of their disease. They support and teach patients with the aim to become independent with their stoma. Working collaboratively with and referring to specialist care services where appropriate. There is a need for close working with the staff from the colorectal surgical wards, outpatient department, endoscopy suite and other specialist nurses working within the Trust and our community stoma service. There is a requirement to travel to other hospital sites and it may also be required to travel to patient’s homes. The Colorectal and Stoma Nurse acts primarily as a resource for all those involved in the care and management of the patient with a diagnosis of colorectal disease. They facilitate the links between the Trust and community, working with the Primary Care Groups, to provide a co-ordinated system of referral, which will ensure that continuity of care takes place
