An exciting opportunity has arisen for a qualified Senior Social Worker to join our Locality Social Work team based at Bullenshaw House, Hemsworth. This is a full time, permanent position (37 hours, Mon - Fri).
Our team work around the East of the Wakefield district, alongside individuals aged 18+ who have care and support needs arising from their physicaland/or mental health. We support individuals and their family with identifying their goals and needs in a Care Act, person centred way, helping them to become more independent and importantly participate in the community in which they live.
We are seeking an individual who is passionate about adult social care and providing a high-quality service and is keen to develop their professionalexperience and knowledge further. You will play a key part in delivering our adult social care strategy and will be committed to strengths-based and outcome-focused practice.
As a member of the team, you will ensure that the service delivers positive, strength-based outcomes for people. You will have a strong commitment to human rights; you will ensure the person’s voice is heard and acted upon.

About the role
* As a Social Worker within the team, you will be:
* Managing a complex but manageable caseload.
* Supporting colleagues and student Social Workers as required.
* Undertaking Care Act assessments, Carers Assessments, co-producing care and support plans and reviewing peoples identified outcomes.
* Completing Mental Capacity Assessments and Best Interest Decision making.
* Part of a duty rota within core hours.
* Working with a range of teams across the Council and our partners including primary and secondary health services, voluntary and private sector services.
