Greater Manchester ICB - Bury operates a Continuing Healthcare/Complex Case service on behalf of the registered population of Bury. The postholder will be responsible for undertaking Specialist Health Needs assessments for individuals who are undergoing assessment for, or who are already eligible for NHS Continuing Healthcare and NHS Funded Nursing Care. Assessments may take place in an individual's own home, residential placements, or hospital environments.

Main duties of the job
The postholder will undertake Specialist Health Needs assessments for individuals eligible or being assessed for NHS Continuing Healthcare and NHS Funded Nursing Care.
They will work with individuals, their representatives, private providers, partner organisations, and other health and social care professionals to ensure assessments are person-centered.
The postholder will work flexibly, supporting the Individualised Commissioning team by participating in the 'Duty' rota, handling urgent referrals, and safeguarding investigations.

Job responsibilities
The postholder will conduct health needs assessments for individuals receiving funded care through GM ICB Bury, liaising with patients, families, and carers involved in commissioned support and reviews.
They will promote service user involvement, collaborate with other agencies, and provide guidance and supervision to staff conducting assessments, ensuring consistency and compliance with national policies.
Where necessary, they will perform independent clinical assessments of individuals with complex healthcare needs, support the Head of Service in identifying eligibility for funding, and negotiate care delivery with NHS and external providers.
The postholder will support Adult Safeguarding meetings, ensure reviews of eligible service users, and take steps to ensure contractual adherence by service providers. Accurate record-keeping and assessments are essential.

* Familiarity with national and local NHS CHC/FNC guidance
