Home Visiting ACP/ ANP/ Prescribing Paramedic

Join Ash Grove Medical Centre as a Home Visiting Advanced Clinical Practitioner (ACP), Advanced Nurse Practitioner (ANP) or Prescribing Paramedic and become part of a supportive, forward thinking GP training practice committed to delivering exceptional community based patient care. This is a rewarding opportunity for an experienced autonomous clinician who is passionate about frailty, care home medicine and palliative care to make a genuine difference to patients within their own homes and residential settings.

The post holder will provide advanced clinical care to housebound patients, care home residents, frail patients and those requiring palliative and end-of-life support within the community. The role includes regular care home ward rounds, urgent and routine home visits, proactive frailty management, Gold Standards Framework (GSF) reviews, ReSPECT documentation and prescribing of palliative and anticipatory medications where appropriate.

The successful candidate will work both autonomously and collaboratively within the multidisciplinary team, using advanced clinical assessment and decision making skills to deliver safe, high quality, patient centred care.

The role is primarily community based however, the post holder may also be required to support surgery based clinics and appointment as and when necessary to meet service demands.


Main duties of the job

Key responsibilities include:

* Undertaking weekly ward rounds within care homes
* Completing home visits for acute, chronic, frail housebound patients
* Undertaking clinical sessions within the surgery as and when needed
* Managing patients with complex health needs using advanced clinical assessment and diagnostic skills
* Supporting palliative and end-of-life care planning, including Gold Standards Framework (GSF) reviews and ReSPECT documentation.
* Prescribing and reviewing palliative and end-of-life medications in line with national and local guidance.
* Working proactively with patients, relatives, carers and community teams to ensure high quality care.
* Providing holistic assessment, diagnosis, treatment and management plans
* Prescribing medication appropriately and safely within scope of practice
* Preventing avoidable hospital admissions through proactive management and early intervention.
* Maintaining accurate and contemporaneous clinical records using SystmOne
* Participating in multidisciplinary meetings, safeguarding processes, significant event reviews and clinical audits.
* Supporting quality improvement initiative and QOF targets where appropriate.

Job responsibilities

Main Duties & Responsibilities

* Undertake regular home visits for acute, chronic, frail, palliative, and housebound patients
* Carry out weekly ward rounds within nursing and residential care homes
* Assess, diagnose, treat, and manage patients with complex and undifferentiated conditions
* Undertake proactive frailty assessments and care planning
* Support palliative and end-of-life care management, including Gold Standards Framework (GSF) reviews
* Complete and review ReSPECT forms and advance care planning documentation
* Prescribe and review medications safely and effectively, including anticipatory and palliative care medications
* Work collaboratively with GPs, district nurses, pharmacists, hospice teams, community matrons, social care, and secondary care services
* Prevent avoidable hospital admissions through proactive management and early intervention
* Support continuity of care for vulnerable and high‑risk patients
* Undertake urgent same‑day clinical assessments where required
* Maintain accurate, contemporaneous, and legally compliant patient records using SystmOne
* Participate in multidisciplinary team meetings, safeguarding reviews, clinical audits, and quality improvement initiatives
* Support the practice in achieving QOF, enhanced service, and CQC quality standards
* Provide support, supervision, and advice to colleagues, trainees, and students where appropriate
* Support surgery clinics and practice-based appointments as and when required to maintain safe service delivery
* Carry out duties requested by the GP Partners and management team in a professional and timely manner

Clinical Responsibilities

* Work autonomously within professional scope of practice to assess, diagnose, plan, implement, and evaluate treatment for patients presenting with acute, chronic, complex, and undifferentiated conditions
* Conduct advanced clinical assessments within patients homes, care homes, and surgery settings where required
* Perform physical and psychological assessments and interpret findings to support clinical decision‑making
* Diagnose and manage acute illness, chronic disease, frailty syndromes, and palliative care needs
* Develop holistic, evidence‑based treatment and management plans tailored to individual patient needs
* Prescribe medications appropriately and safely within professional scope of practice and national/local prescribing guidance
* Prescribe and review palliative and anticipatory medications for end‑of‑life patients
* Identify deteriorating patients and escalated care appropriately
* Support admission avoidance and facilitate safe community management
* Order, interpret, and act upon diagnostic investigations where appropriate
* Support patients with medication adherence, lifestyle advice, self‑management, and preventative healthcare
* Deliver care aligned with NICE guidance, local pathways, and evidence‑based practice
* Recognise and respond appropriately to safeguarding concerns for children and vulnerable adults
* Work within professional boundaries and escalated appropriately outside own competence

Multidisciplinary Working

The post holder will:

* Work collaboratively with GP Partners, nursing staff, pharmacists, care coordinators, social prescribers, district nurses, hospice teams, community services, and external agencies
* Participate in MDT meetings including frailty and Gold Standards Framework meetings
* Liaise effectively with secondary care and community providers to ensure continuity of care
* Support discharge planning and coordinated patient management
* Contribute to service development and quality improvement initiatives

Professional Responsibilities

The post holder will:

* Maintain accurate and contemporaneous clinical records and documentation
* Participate in annual appraisal, clinical supervision, and continuing professional development
* Maintain up‑to‑date knowledge of evidence‑based practice and clinical guidance
* Participate in audit, reflective practice, and benchmarking activities
* Raise and document significant events, incidents, and near misses appropriately
* Support practice governance, patient safety, and quality improvement processes
* Maintain confidentiality in accordance with GDPR, Caldicott principles, and practice policies
* Comply with infection prevention and control procedures and safe clinical waste disposal
* Maintain professional registration and revalidation requirements
* Support the practice in maintaining high CQC standards and compliance
* Participate in rota cover during periods of annual leave or sickness absence
* Support the reception and administration team with clinical queries where appropriate
* Maintain a professional appearance and conduct at all times
* Ensure patient identifiable information is stored securely in accordance with data protection legislation
* Attend mandatory training and complete updates as required
* Participate in local and national CPD relevant to the role
* Support students, trainees, and colleagues within the practice as appropriate
* Contribute to clinical audits, research projects, and service improvement initiatives

Organisational & Statutory Requirements

The post holder must comply with the Health & Safety at Work Act 1974 and all practice policies relating to:

Other Information

* 30 days annual leave plus bank holidays (pro rata for part‑time employees)

Knowledge and Skills

* Excellent clinical assessment and diagnostic skills
* Sound knowledge of evidence based practice and national clinical guidance

Qualifications

* Registered Advanced Clinical Practitioner, Advanced Nurse Practitioner or HCPC Registered Paramedic.
* Current professional registration with NMC or HCPC
* Masters degree in Advanced Clinical Practice or equivalent advanced clinical qualification.
* Evidence of continued professional development
* Full UK driving licence and access to a vehicle for work purposes.
* Qualification or experience in frailty or palliative care
* Non‑medical prescribing updates or advanced clinical skills courses
* Gold Standards Framework (GSF) or end of life care training


Experience

* Significant experience working autonomously within primary care, urgent care, community or care home settings
* Experience managing patients with acute and chronic conditions
