Band 6 Memory Nurse – EMDASS Team
An exciting opportunity to join our Countywide EMDASS team as a Band 6 Memory Nurse, delivering services across North Hertfordshire.
Key Responsibilities
* Conduct memory assessments for service users suspected of dementia.
* Deliver dementia diagnosis in collaboration with multidisciplinary teams.
* Provide post‑diagnostic treatment and support through titration reviews.
* Work between the One‑Stop Pathway and Primary Care to manage patient pathways.
* Carry out risk assessments and implement risk management plans with service users and carers.
* Ensure safeguarding policies are followed, maintaining the safety of service users and carers.
* Co‑ordinate referrals to partner agencies and community services tailored to individual needs.
* Act as Duty Nurse for screening referrals, offering advice and sign‑posting to appropriate services.
* Supervise junior staff and trainees as delegated by the EMDASS Team Leader.
* Support the transformation of the EMDASS service by improving pathways and procedures to enhance service efficiency.
Essential Skills & Experience
* Excellent organisational and communication skills.
* Robust clinical experience in dementia assessment and care.
* Ability to work independently, solve problems, and maintain accurate records.
* Strong clinical leadership to support students and junior staff.
* Flexibility to meet evolving service needs.
