The Nursing Team deliver clinical evidence-based practice for our local population, meeting the needs of the patients, with particular attention to patients with long term conditions and the provision of preventative healthcare.
Main duties of the role may include:
* Chronic Disease Management including COPD, Asthma and Diabetes
* Adult and Child Immunisations
* Cervical Cytology
* Wound care
* Contraceptive reviews
* General Health Advice
Main duties of the job
Working independently using their own initiative and accountable for their own professional actions.
Administers adult and child immunisations & vaccinations in accordance with national & local programmes. Competent in anaphylaxis & resuscitation techniques. Ensures safe storage of vaccines & ensures cold chain maintained.
Undertakes procedures to include venepuncture, BP management, urinalysis, peak flow, pregnancy testing, stitch/clip removal, ear syringing, cytology, wound management.
Recognises and understands the importance of observing, documenting & communicating the signs & symptoms of child & adult abuse.
Undertakes chronic disease annual reviews within the practice in order to meet the requirements of QoF promoting self help and management.
Initiates the process of diagnosis with patients suspected to have a chronic disease eg diabetes, COPD, asthma, referring to other clinicians as appropriate.
Participates in clinical audits and research studies.
Refers to other members of the multi-disciplinary team.
Job responsibilities
KEY ROLE CLINICAL NURSING PRACTICE
The Practice Nurse works independently using own initiative and is accountable for their own professional actions.
Administers adult and child immunisations and vaccinations in accordance with national and local programmes. Is competent in anaphylaxis and resuscitation techniques. Ensures safe storage of vaccines and ensures cold chain maintained.
Undertakes procedures to include, venepuncture, BP management, ECGs, urinalysis, peak flow, pregnancy testing, stitch/clip removal, ear syringing and cytology, wound management.
Able to recognise a deteriorating patient and seek assistance when required.
Recognises and understands the importance of observing, documenting and communicating the signs and symptoms of child and adult abuse.
Works within all relevant practice and CCG policies and procedural guidelines eg infection control, chaperoning and patient group directions.
KEY ROLE CARE AND PROGRAMME MANAGEMENT
Undertakes chronic disease annual reviews within the practice in order to meet the requirements of QoF promoting self help and management.
Initiates the process of diagnosis with patients suspected to have a chronic disease eg diabetes, COPD, asthma, CHD referring to other clinicians as appropriate.
Participates in clinical audits.
Refers to other members of the multi-disaplinary team.
KEY ROLE HEALTH PROMOTION
Initiates and carries out programmes of health screening.
Enables, encourages and supports individuals, and families to address issues which affect their health and social well-being.
Represents individuals and families interests when they are not able to do so themselves.
Provides healthy lifestyle and smoking cessation advice and signposts to appropriate support services.
Makes referrals to statutory and voluntary agencies as appropriate.
KEY ROLE MANAGE SELF
Contributes to the development, monitoring and evaluation of care delivery by the Nursing Team.
Promotes effective communication and relationships within the team.
Works collaboratively as a team member to achieve the objectives
Partakes in clinical supervision for own development.
Recognises the development needs of others and provides clinical supervision and mentorship to HCAs
Actively participates in clinical and practice meetings.
Develops own knowledge and practice to meet objectives/changes in service.
Experience
* Works in accordance with the NMC Code of Professional Conduct and The Scope of Professional Practice.
* Previous experience in General Practice / Primary Care
* Experience of working in a busy environment
* Able to use EMIS Clinical System
Qualifications
* NMC Registered general Nurse
