The Home First Neighbourhood Vale Team is part of Oxfordshire County Council’s Adult Social Care, revolutionising support for those returning home after a hospital stay. This collaborative reablement service works closely with local NHS trusts and other partners to ensure residents’ needs are assessed and met in their own homes, not just at the hospital bedside.

About the Role

This role encompasses the Vale area of Oxfordshire, including Faringdon, Grove, Wantage, and Abingdon. It is a full-time position of 37 hours per week, Monday to Friday, with participation in an on-call weekend rota.

The successful candidate will collaborate with colleagues across Adult Social Care and partner agencies to support and strengthen communities, delivering meaningful impact for both current and future populations, while ensuring value for money. Support will be provided through a strengths-based approach designed to create opportunities, foster resilience, and build support networks, enabling individuals to live as independently, safely, and successfully as possible.

The post holder will engage with colleagues and key stakeholders across organizational boundaries, prioritizing safe, effective, and prompt discharges via a Discharge to Assess (D2A) model. Responsibilities include assessing patients in the community within 72 hours of hospital discharge and promoting long-term independence.

Working in close partnership with reablement providers and the voluntary sector, the post holder will guide and support individuals in regaining independence at home, utilizing a strengths-based approach to goal planning during short-term intermediate care. This intervention will be delivered following hospital discharge or to prevent unnecessary admissions.

The ideal candidate will leverage skills in activity analysis, assistive technology, aids, and minor adaptations to help reduce reliance on formal care wherever possible.
