Memory Assessment Nurse

The Managing Memory Together Service is a dynamic service, continually developing in response to the latest dementia policy drivers, both nationally and locally. As a Memory Assessment Nurse you will be part of a team that develops skills in assessment and treatment options for people living with dementia, building working relationships within primary care and community services.
Responsibilities
* Carry out comprehensive cognitive and mental health assessments to support the diagnosis of dementia.
* Triage referrals and recognise where non‑organic factors may contribute to memory problems, ensuring these are managed before a dementia assessment is considered.
* Liaise with partner agencies such as the Alzheimer’s Society Dementia Advisor Service, Adult Social Care and primary care colleagues to provide specialist advice and support in the diagnosis of dementia, escalating concerns where required.
* Provide post‑diagnostic support to people with dementia and their carers, and contribute to patient and carer education group sessions within Managing Memory Together.
Main duties
The Memory Assessment Service receives upwards of 200 referrals a month countywide. The post holder will conduct cognitive assessments for patients referred into the memory assessment pathway by GPs and other healthcare professionals, ensuring referrals are appropriate and reversible causes for memory difficulties have been excluded. The post holder will also carry out nurse diagnosis appointments where appropriate, provide post‑diagnostic support to patients and carers, and help facilitate regular patient and carer education sessions alongside the Information and Education Service.
Qualifications and Experience
* Experience working with older people with dementia and other mental health problems.
* Working knowledge of the complex needs of older people.
* Understanding of possible symptoms of dementia and its most common types.
* Strong clinical assessment skills, including mental state examination, history taking and risk assessment.
* Excellent written and verbal communication skills, with experience in report writing.
* Experience completing dementia assessments using cognitive tests.
* Ability to manage own caseload and prioritise tasks.
* Experience supporting group work or other teaching.
* Experience working in a multi‑disciplinary team and with other agencies.
* Experience applying psychosocial interventions with people with dementia.
* Sound evidence‑based knowledge of the management of people with dementia and mental health needs.
* Sound knowledge of person‑centred care and principles of social inclusion.
* Awareness of legal and statutory frameworks including the Mental Capacity Act, Mental Health Act and Safeguarding.

Salary: £32,073 to £39,043 a year (pro rata)
