Overview
We are looking for an enthusiastic and motivated Physical Health Support Worker to join our busy and friendly team. The Later Life Team supports Adults over the age of 65 years who have secondary mental health needs, both organic and functional as well as carrying out Memory Assessments. We cover the South Manchester area and support people living in their own homes, sheltered/Extra Care Housing or in care homes.
Please note - This role is not eligible for sponsorship unless you meet criteria by temporary exemption from current changes to immigration rules put in force on 22/07/2025. Separate provisions are applied to workers who have been sponsored and held continuously Skilled Worker visa since prior to 04/04/2024.
Responsibilities
* Complete physiological assessments of our service users, including taking and recording physical observations such as blood pressure, pulse, and weight, as well as Venepuncture. Training will be available for the right candidate.
* Support the Memory Treatment pathway by undertaking relevant observations.
* Assess service users holistically and complete Physical Health Assessments. Visits will take place in service users' homes.
* Be an active and contributing member of the Community Mental Health Team and feedback to MDT as needed.
* Provide information on health promotion which may include smoking cessation, alcohol use, dietary needs appropriate to our client group.
* Professional development will be supported by the service through regular supervision and training opportunities.
