Accommodation and Community Support Worker
As part of a team, provide effective 24-hour support to clients accessing our services, such as hostels, family, and remote support units. Please see the employer website for full job details & person specification.
Responsibilities
* Ensure the needs of service users are effectively met through promoting independence via support, development, and progression.
* Provide support and advice to service users.
* Undertake assessments of service users’ needs and risks to determine the required level of intervention.
* Develop focused, outcome-based support plans and risk management plans, and monitor their delivery.
* Liaise with external agencies as appropriate to ensure coordination, integration of services, and to maximize opportunities.
* Act as an advocate, supporting clients; attend appeals or tribunals; liaise with GPs, healthcare professionals, statutory, and voluntary agencies.
Skills and Qualifications
* Ability to demonstrate the core values of the Simon Community.
* IT skills—ability to use email, internet, and databases to electronically record support work.
* NISCC Registered (or willing to become registered if appointed) and willing to undergo Access NI checks. Successful applicants must start the NISCC registration process prior to employment.
* Six months’ recent experience (paid or voluntary) supporting people with complex and multiple needs within a residential or community setting within the last three years.
* Experience in identifying support needs, including client-specific risks, and developing plans to support those needs.
* A Degree in Social Work or current studies towards a Degree in Social Work.

Work schedule: Three 12.5-hour shifts per week, including days and waking nights.
