Service Support Worker
Location: Taunton, travel within the area
Salary: £22,932 per annum, pro rata (£13,759 at 21hrs per week)
Hours: 21 per week
Contract type: Permanent
Driving: Full UK driving licence and vehicle required

A day in the life of a Support Worker includes:
* Taking patients home from hospital.
* Supporting individuals with basic resettlement upon their return home, ensuring they have access to food, water, and heat.
* Building positive relationships with service users, their families, and healthcare professionals.
* Ensuring all care documentation is accurate and up to date.
Requirements for success
* Previous experience in a similar care role, preferably within healthcare or social care.
* Compassionate and empathetic nature, with a strong desire to make a positive impact.
* Excellent communication and interpersonal skills, effective within a multidisciplinary team.
* Knowledge of person-centred care principles, upholding dignity and autonomy.
* Ability to work independently, demonstrating initiative and reliability.
* Flexibility to work various shifts, including evenings, weekends, and bank holidays.
* A full UK driving license is required.
