Home first is the ideal outcome for many patients leavingthe hospital and providing more care in a persons own home is preferred, but atemporary stay in a setting that that can provide 24-hour care is sometimesrequired. The Recovery Hubs does this, enabling early discharge from the hospitaland reducing hospital admissions. The hub delivers complex rehabilitative carefor the older adults in Leeds and the nursing care is delivered alongside awider multi-disciplinary team. The focusalways is on the individual, helping them progress to going home.
We provide training, clinical supervision, and access to a peer supportnetwork. We won't just stop there though, we are proud of our careerprogression pathways that will allow you to develop your clinical, leadership,and preceptorship skills.
Wharfedale Recovery Hub and the communitycare beds Recovery Hub service works on a shift pattern and covers 24 hours, 7days a week.
Main duties of the job
Communitynursing in the recovery hub suits people who can work independently and withinthe wider multi-disciplinary team. Youll be resilient, motivated, andadaptable to working in an ever-changing environment and of course, a passionfor providing the best possible care to all.You will have great communication skills and maintain a high standard ofclinical skills when carrying out nursing procedures. You will ensurecompliance with our clinical governance framework, you will motivate colleagues,champion service user safety and dignity and drive continuous improvement.
