As a Clinical Pharmacist, you will work as part of a multidisciplinary team to provide expert medication reviews, support long-term condition management, and contribute to safer, more effective prescribing across our practices. You'll play a pivotal role in improving patient outcomes through structured medication reviews, managing repeat prescribing, and supporting clinicians with medicines optimisation.


Main duties of the job

* Conduct structured medication reviews with patients to optimise treatment and reduce risk.
* Support management of long-term conditions through evidence-based prescribing.
* Provide expert advice on medicines to clinicians and patients.
* Contribute to prescribing audits, safety initiatives, and formulary adherence.
* Work closely with GPs and the wider clinical team to enhance prescribing quality.


About us

We are excited to expand our pharmacy team and are currently recruiting for two Clinical Pharmacists and one Pharmacy Technician to join our ever-growing and dynamic workforce.

Based in Devon, with primary sites in Plymouth or Barnstaple, these roles offer the opportunity to work both on-site and remotely to support our wider network of practices across the region.

These are full-time roles (up to 37.5 hours per week), though we are happy to discuss flexible or part-time working arrangements that support work-life balance.

As part of our collaborative team, you will play a key role in delivering high-quality medicines optimisation, supporting long-term condition management, and contributing to safe, effective prescribing within our primary care network (PCN).

Job responsibilities

Medication Management:

Regularly review patient medications to optimise therapeutic outcomes and minimize risks of adverse effects. Adjust prescriptions as necessary in consultation with the Senior Clinical Pharmacist and other healthcare providers.

Prescription: Ensure prescription requests, acute and routine, are prescribed appropriately.

Drug Interactions and Contraindications: Vigilantly monitor for potential drug interactions and contraindications to prevent medication errors and ensure patient safety.

Discharge Resolutions: Resolve medication issues and requests from hospital correspondence.

Pharmacy Tasks: Complete a wide range of medicines-management pharmacy tasks daily.

Patient Care and Counselling:

Provide comprehensive counselling to patients on medication use, side effects, interactions, and storage, enhancing patient knowledge and compliance.

Clinical Decision Support: Assist healthcare providers in making informed clinical decisions regarding pharmacotherapy, utilising up-to-date clinical data and evidence-based guidelines.

Chronic Disease Management: Participate in managing chronic diseases by providing pharmacotherapy expertise, contributing to care plans, and monitoring progress.

Collaboration and Communication:

Work closely with a multidisciplinary team including doctors, nurses, and other healthcare professionals to ensure coordinated patient care.

Support healthcare team members with drug information and pharmacological advice, supporting safe and effective treatments.

Engage with patients to gather health status, medication history, and treatment responses to inform care decisions.


Person Specification


Qualifications

* MPharm degree required or equivalent. Registration as a pharmacist with the GPhC or equivalent is mandatory.


Experience

* Strong clinical pharmacology knowledge and practical application in patient care.
* Proficiency in using Electronic Health Records (EHR) and pharmaceutical software.
* Understanding of pharmacy regulatory standards, GDPR, medication safety, and clinical governance.
* Excellent communication skills for discussing treatment plans and medication info effectively.
* Patient-centred approach, assessing needs and individual responses.
* Analytical and problem-solving skills for evaluating patient info and treatment outcomes.
* Interpersonal skills for effective team collaboration and building relationships.
* Willingness for professional development and participation in research and guideline development.

Pay dependent on experience, in line with PCN range.
