About You:
* Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
* Experience as a pharmacist, demonstrated within a practice portfolio.
* An awareness of common acute and long-term conditions.
* May hold or be working towards an independent prescribing qualification.
* Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate.
* Excellent interpersonal and communication skills, and be organised, patient and empathetic.
Main Duties:
* Undertake Long-term Condition Clinics (COPD/Asthma) and Medication Reviews
* Conduct Patient Facing Medication Reviews including Care Home/Domiciliary and F2F
* Manage a caseload of patients with common/minor/self-limiting ailments
* Make recommendations for medicine optimisation and safety.
* Support staff with prescription and medication queries as well as manage repeat prescriptions and monitoring tests.
* Lead medicines optimisation and quality improvement.
* Provide training on therapeutics and medicines optimisation and critically evaluate literature and ensure compliance with CQC standards
