Physiotherapy assistant practitioner - Acute Medicine

Nottingham University Hospital's NHS Trust (NUH) has an opportunity for an enthusiastic, dynamic and highly motivated individual to join the Acute Medicine Physiotherapy Team at Queen's Medical Centre (QMC) working as a Band 4 Physiotherapy Assistant Practitioner, part time (18.75h per week), on a 12 month fixed term contract or secondment.

Acute Medicine is a fast paced speciality with a high acuity and turnover of patients.

You will be expected to manage your own caseload, perform new patient assessments and formulate appropriate discharge plans to support patient flow through the hospital.

This role primarily involves working on the front door, on Ward B3 and the Acute Medical Receiving Area (AMRA). There will be some opportunity to support the physiotherapy team on the Level 1 Unit with the early mobilisation and rehabilitation of patients in the acute phase of their hospital journey.

You will have support from qualified staff with complex patients and the opportunity to have regular supervision.


Main duties of the job

Managing a caseload of patients on a short stay medical ward, with support from a qualified team of physiotherapists.

Assessing patient's mobility and suitability for discharge. This caseload normally consists of frailty, falls, and care home patients as well as less complex medical patients requiring mobility assessments.

Participating in delegated rehabilitation intervention. On B3 and AMRA there may occasionally be patients with simple upper and lower limb trauma, ambulatory stroke/ neurology patients and respiratory patients. These will be assessed by a qualified physiotherapist and you may have some involvement in reviewing their progress/ supporting with rehabilitation.

Attending multidisciplinary board round with a focus on flow and discharge and an opportunity to be an advocate for patients.

Ordering and collecting mobility aids to ensure sufficient supply.

Accurately recording patients contacts on clinical manager.

Updating Nervecentre with a summary of the patients mobility status and discharge plans and sending referrals in a timely manner.

Sending community referrals to community partners for patients at risk of falling or with a reduction in mobility/function.

The Physiotherapy service is part of the Therapies, Outpatients and Phlebotomy Services (TOPS) Directorate, within the Specialist Clinical Services care group. We are a diverse directorate of AHPs, nursing and support staff, employing around 1000 staff, working together to deliver the high quality care we aspire to. Our HCPC registered AHP professionals comprise of physiotherapists, dietitians, occupational therapists, orthotists, prosthetists and speech and language therapists. We have a Chief AHP to lead us on our strategic direction, and many of the directorate leadership team have AHP backgrounds to support our staff. There are also Practice Development Therapists to support career development for all.

Job responsibilities

The Acute Medicine Team is part of the wider Acute Inpatient Team, which covers Critical Care, Surgical and General Medicine wards. There are regular opportunities to attend in-service training sessions, with topics and guest speakers from a wide variety of specialities. You will have regular monthly supervision sessions and an annual appraisal.

On the Acute Medical wards you will work closely with the Front Door Integrated Discharge Team (IDT), Nurses, Doctors and Advance Clinical Practitioners. This is a very supportive environment and there is a real focus on patient safety and discharge where the input of therapy staff is highly valued and appreciated.

Role is part time, 18.75h per week working Wednesday-Friday. Core hours are 8.15-4.15.


Person Specification


Training and qualifications

* Level 2/GCSEs grade 4 or higher English and maths (Grade C or above prior to 2017) or equivalent Functional Skills
* NVQ/QCF level 3 in healthcare or equivalent relevant qualification
* Foundation degree in Health and Social Care, or equivalent level 5 qualification


Experience

* Significant experience of working as a therapy support worker (band 3) (band 3)
* Able to deal with pressurised/unexpected situations
* Has experience of working in a rehabilitation or relevant healthcare setting
* Previous experience of working as a Band 4 assistant practitioner


Communication and relationship skills

* Effective written, verbal and non-verbal communication skills
* Able to communicate complex information with patients who have impaired communication abilities
* Able to work effectively as a team player as part of an interdisciplinary team


Analytical and judgement skills

* Demonstrates a responsible attitude/professional behaviour
* Able to work flexibly as part of a team
* Demonstrates problem solving skills and ability to work on own initiative


Planning and organisation skills

* Flexible and willing to adapt


Physical skills

* Able to undertake therapeutic moving and handling procedures
* Experience of using Information Technology (e.g. basic word processing skills).


Other

* Flexible with working requirements. Working patterns will be dependent on each clinical setting and may be over 7 days.
