About the Children and Young People's Learning Disability Service (CLD)
CLD is part of the Learning Community within the Mindworks contract at Surrey and Borders Partnership NHS Foundation Trust (SABP) and is seeking a highly motivated Band 7 Nurse to join our team based in Godalming. CLD is a Surrey-wide, nurse‑led community service. Our multidisciplinary team includes Learning Disability Nurses, Assistant Psychologists, Psychiatrists, Music Therapists, and our dedicated administrative staff.
We offer twice‑monthly Clinical Issues Meetings, monthly Business Meetings, and a quarterly Learning Disability Forum. Regular supervision and appraisal are provided, alongside ongoing opportunities for career development.
Key Responsibilities
* Provide supervision and appraisals for Band 6 colleagues, and offer clinical advice and case management support.
* Hold a group of allocated young people and work with them and their network to support their presenting needs.
* Support the ASD, ADHD, and behavioural assessment pathway, including observations, history gathering, and case discussion.
* Act as practice assessor/teacher to pre‑registration students and junior personnel and participate in the audit of educational placements.
* Demonstrate specialist nursing knowledge in learning disability, crisis intervention, alternative communication, assessment and management of additional mental health needs, epilepsy, medication management, challenging behaviour, and related conditions.
* Actively involve children and families in developing and agreeing their own care plan.
* Advocate and ensure respect for children's dignity, choice, self‑esteem, racial, cultural, sexual and religious needs and preferences.
* Practice within the Positive Behaviour Support framework and undertake comprehensive risk assessments, developing effective risk management plans.
* Work in partnership with acute and primary care services and other healthcare providers.
* Maintain effective relationships with colleagues, agencies, children, young people and families.
* Support the lead nurse in delivering nursing provision within the service and provide clinical supervision to junior staff.
* Participate in peer supervision within the CYPS Learning Disability Team.
* Ensure standards of service provision are followed, monitored and maintained.
* Assess, plan, implement and evaluate the nursing care needs of children and young people with learning disabilities who use our services.
* Hold a complex caseload with a variety of nursing needs.
* Supervise and teach unqualified staff including students and work as part of a multi‑disciplinary team supporting service users and their carers.
