Eden Brown Synergy is currently recruiting an experienced Qualified Social Worker (QSW) to work within Continuing Healthcare (CHC) for Adult Services in Lancashire.
This role is ideal for candidates with strong CHC experience who can confidently complete assessments and Decision Support Tools.
Continuing Healthcare Social Worker (QSW)
Adult Services - Health & Wellbeing
Location: Lancashire
Rate: £35 per hour
Essential Requirements
Qualified Social Worker with relevant post-qualification experience
Proven experience completing NHS Continuing Healthcare assessments
Strong knowledge and hands-on experience with Decision Support Tools (DSTs)
Good working knowledge of Liquid Logic
Ability to work autonomously and manage a varied caseload
Please note: Only candidates with direct CHC and DST experience will be considered.
