Look no further! We’re thrilled to announce an opening for a CPN to join dynamic community team in Bristol.
As a CPN, you’ll be an integral part of our multidisciplinary team, working closely with individuals dealing with various mental health challenges. Your role will involve:
- Conducting assessments and developing personalised care plans.
- Providing therapeutic interventions and support to patients in their homes and the community.
- Collaborating with psychiatrists, psychologists, social workers, and other healthcare professionals.
- Monitoring and evaluating the progress of patients and adjusting care plans as needed.
- Offering guidance and education to patients and their families.
Qualifications: To excel in this role, you’ll need:
- Valid registration as a Mental Health Nurse with the Nursing and Midwifery Council (NMC).
- Experience working in a community mental health setting.
- Excellent communication and interpersonal skills.
- Strong assessment and care planning abilities.
- Empathy and a non-judgmental approach to patient care.
- Clear driving licence and own a car
