Senior Community Practitioner

An exciting opportunity has arisen for a kind and experienced Senior Community Practitioner to join our friendly and dedicated colleagues at Princess Road Clinic in Malton.
Whitey and Ryedale Integrated Community Team provides mental health services and delivers care to adults across the Whitby and Ryedale areas through two team bases. Our second office is based in Whitby.
As a member of the team, you will work alongside all members of the multi‑disciplinary team which includes Registered Nurses, Social Workers, Psychologists, Psychology Assistants, Consultant Psychiatrists and medics, Pharmacists, Team admin and support workers. You will have access to and support from specialist services available within the Trust, and excellent working relationships with local partner organisations.

Main duties of the job
As a named worker/care coordinator you will be expected to hold a case load of patients, to be able to deliver care to meet the identified patients needs autonomously and with the support from the MDT. You will be required to participate in the duty worker rota and support the patient journey from referral to discharge. As a senior community practitioner, you will be expected to provide support to junior members of the team and to provide an excellent learning environment for students who are placed with the team.
Candidates must have registration with the relevant governing body and 18 months post‑qualifying experience including sign off of appropriate preceptorship or competency framework.
Person Specification
Qualifications
* Current professional registration with an approved professional body.
* Evidence of Continuing Professional Development.
* For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale.
* 18 months post‑qualifying experience including sign off of appropriate preceptorship or competency framework.
* Recognised sign‑off mentor, clinical educator or equivalent. Must be achieved within agreed timescale.
* Key skills in literacy, numeracy and ITQ level 2 (or equivalent).
* Post‑graduate qualification in a relevant area.
Experience
* Significant experience working with people with mental ill health in a care environment.
* Working collaboratively with service users and their families/carers.
* Providing clinical supervision to individuals or groups as professionally appropriate.
* Mentoring or facilitating students on practice placement; working in a multi‑disciplinary team.
* Significant experience working with mental ill health in a community environment.
Knowledge
* Clinical Risk Assessment and Management and its application in practice.
* Understanding of psychological models of care and treatment.
* Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act).
* The Trust's Quality Improvement System (QIS).
Skills
* Communicate complex and sensitive information effectively to patients, carers/families and all members of the multidisciplinary team.
* Work effectively as part of a multidisciplinary team and undertake lead professional/care co‑ordinator responsibilities.
* Provide effective clinical supervision, teaching, training and assessing in clinical practice.
* Competency in venepuncture.
