Job summary
To work underthe direction of the Clinical Lead Outreach Services in aspects of management,leadership and co-ordination of the Palliative Care Virtual Ward. Ensuring high standards of patient care aremaintained in accordance with all St. Roccos Hospice policies, procedures, andguidelines.
Main duties of the job
OPERATIONALRESPONSIBILITIES
Totake responsibility of the virtual ward caseload and the associated day-to-daywork allocation and management duties.
Toundertake an in-depth holistic first assessment visit, enable service usersto access remote monitoring technology, plan, implement and evaluate care inline with care plan.

Applicants must be on the NMC register and be fully qualified RGN / Adults with minimum 12 months post registration experience, preferably in palliative care. In addition to your proven clinical skills, we are looking for excellent communication skills, empathy and sensitivity, good team working and a flexible approach to the role.
Additional unsociable hour enhancements will be paid for weekend and bank holiday work - T&Cs apply. Shift patterns will be discussed at interview, breaks are unpaid.

To ensure clinical incidents are reported in line with timescales and subsequent actions are disseminated throughout the team and acted upon appropriately.
