Joining our Clinical Pharmacy Team across General Practices and Care Homes in Surrey Heath, playing an important role alongside clinical pharmacists and the multidisciplinary team (MDT). Working under supervision of a clinical pharmacist, undertaking resident/patient-facing activities, and supporting the team to ensure effective use of medicines within care homes and general practice through shared decision-making conversations with residents/patients.
Initially, focusing on supporting the Practice(s) and Primary Care Network (PCN) to implement medicines optimisation requirements of the new Network Contract Direct Enhanced Services and other responsibilities. As the Pharmacy Technician role develops, the focus will expand to support the clinical pharmacy team and meet the needs of the practice(s) and population served.
A key focus will be supporting GPs and clinical pharmacists in implementing effective medicine management in care homes and across the practice population, identifying areas for improvement, and managing change.
Key Responsibilities:
1. Function as part of the general practice and PCN team, working across Surrey Heath practices and care homes, developing professional relationships.
2. Carry out medicines optimisation tasks to improve prescribing quality, cost-effectiveness, patient adherence, and administration.
3. Support structured medication reviews and medication reviews.
4. Build and run EMIS reports to identify and stratify patients/residents for review.
5. Undertake medicines reconciliation, review medication changes, and communicate changes appropriately, including for new care home residents and post-hospital discharge patients.
6. Synchronize medicines during patient transfers between care settings, liaising with community pharmacists and referring to the clinical pharmacist for structured medication reviews.
7. Use consultation skills to work with patients/residents to ensure effective medicine use (e.g., inhaler technique).
8. Provide specialist expertise to address public health and social care needs, including lifestyle advice and tackling health inequalities.
9. Support practice team members on medicines optimisation, prescription requests, queries, and drug monitoring.
