Job responsibilities
Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients. The post-holder will work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners. The post-holder will have the required level of training and competence and will work within patient group directions where these are available. A nurse with prescribing qualifications will use them appropriately. The post will be held at Riverside Surgery, 525 New Chester Road, Rock Ferry, Wirral CH42 2AG. On occasion there may be a requirement to undertake home visits.
Duties and responsibilities:
* Management of chronic disease
* Identifying significant abnormalities
* Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals
* Advanced knowledge in the diagnosis, control and management of Asthma, COPD and Diabetes is essential
* Knowledge in Hypertension, CHD, AF, PAD, Stroke, CKD, Mental Health, Learning Disability, Depression, Rheumatoid Arthritis, Palliative Care and Epilepsy is desirable but not essential as training can be given

* Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; social problems)

Salary
£19 - £22 per hour depending on experience
Person Specification
Qualifications
* 1st Level Registration in Adult Nursing
* Membership of a Professional Body
* Minimum of at least 2 years experience in Practice Nursing, in Primary Care
* Knowledge re EMIS
* Advanced knowledge in the diagnosis and management of Asthma, COPD, Diabetes and other chronic diseases
* Cervical Cytology Screening and Competency
* Immunisations and Vaccinations training for both Adult and Child
* Understanding of QOF
* Demonstrate an understanding of audits
* Good understanding of Health and Safety
* Good understanding of Infection Control
Experience
* RGN Registration in Adult Nursing
* Membership of a Professional Body
* Minimum 2 years experience working in General Practice (Primary Care)
* Advanced knowledge of Asthma, COPD, Diabetes
* Knowledge of all chronic diseases
* Cervical Cytology Screening and Competency
* Adult and Child Immunisations
* A good understanding of QOF
* Understanding of EMIS
* Demonstrate an understanding of audits
