Senior Clinical Pharmacist (Advanced Practitioner)
* This is an exciting new opportunity for an Advanced Clinical Pharmacist (ACP) to develop their career in a primary care setting. Connexus Primary Care Network supports over 50,000 patients registered with five GP Practices in South-East Bristol.
Main duties of the job
To further develop and enhance the clinical pharmacy service for the patients of the Connexus Primary Care Network (PCN). To provide clinical leadership, supervision, and mentoring to Clinical Pharmacists, Pharmacy Technicians, and Clerks working for the PCN. To oversee the General Management (clinical and non-clinical) of the PCN Pharmacy Hub located at Brooklea Health Centre.

The PCN employs Clinical Pharmacists, Pharmacy Technicians, and Pharmacy Clerks, working closely with Practice-employed Pharmacists, GPs, and medicines management leads to ensure high-quality clinical services.

Job responsibilities
Support the PCN and Practices to achieve maximum performance through the Investment and Impact Fund (or equivalent) and QOF, including maintaining good relationships with Community Pharmacy and supporting Practices to meet prescribing targets.
Provide high-quality patient-facing services, including cover for the pharmacy team during annual leave, study leave, or planned sick leave:
o Long-term condition reviews - Manage care for patients with chronic diseases requiring medication optimization, e.g., COPD, asthma.
o Patient-facing clinical medication reviews - Conduct reviews with patients and provide recommendations on prescribing and monitoring to the clinical team. Support patients in medication management, waste reduction, and self-care.
o Develop specialist prescribing expertise aligned with PCN priorities.
Ensure systems are in place for effective care home medication reviews, supporting the Pharmacy Team in medication safety and administration improvements.
Provide expertise in medicines use, addressing public health and social care needs to reduce health inequalities in the Connexus PCN area.
Take a central role in shared care protocols, clinical research with medicines, and liaising with specialist and community pharmacists.

Qualifications
* Mandatory registration with the General Pharmaceutical Council.
* Independent prescribing qualification.
* Minimum of 2 years post-qualification experience.

Salary range: £50,000 - £60,000 depending on experience (Pro Rata).
