Are you an experienced Advanced Clinical Practitionerlooking for a new opportunity in a picturesque rural, coastal setting? We areseeking a dedicated and skilled ACP to join our friendly and dynamic team at MorrabSurgery in Penzance, Cornwall.


Main duties of the job

* Conduct comprehensive patient assessments, including history taking, physical examinations, and diagnostic investigations.
* Diagnose and manage a wide range of acute and chronic medical conditions.
* Prescribe medications and treatments in accordance with practice guidelines and legal requirements.
* Lead and support clinical initiatives to improve patient care and practice performance.
* Mentor and supervise junior staff, including nurses and healthcare assistants.
* Work collaboratively with GPs, nurses, and other healthcare professionals to ensure integrated and coordinated care.

Job responsibilities

Job Summary: The Advanced Clinical Practitioner (ACP) is a highly skilled healthcare professional who provides expert clinical care and leadership within the general practice. The ACP will work autonomously and collaboratively with the healthcare team to deliver high-quality, patient-centred care, focusing on assessment, diagnosis, treatment, and management of patients with acute and chronic conditions.

Key Responsibilities:

* Clinical Practice:
* Conduct comprehensive patient assessments, including history taking, physical examinations, and diagnostic investigations.
* Diagnose and manage a wide range of acute and chronic medical conditions.
* Prescribe medications and treatments in accordance with practice guidelines and legal requirements.
* Provide expert clinical advice and support to patients, families, and carers.
* Leadership and Management:
* Lead and support clinical initiatives to improve patient care and practice performance.
* Mentor and supervise junior staff, including nurses and healthcare assistants.
* Participate in clinical governance activities, including audits, risk management, and quality improvement projects.
* Education and Training:
* Provide education and training to healthcare staff, promoting continuous professional development.
* Stay updated with the latest clinical guidelines, research, and best practices.
* Participate in personal and professional development activities to maintain and enhance clinical skills.
* Collaboration and Communication:
* Work collaboratively with GPs, nurses, and other healthcare professionals to ensure integrated and coordinated care.
* Communicate effectively with patients, families, and the healthcare team to ensure clear understanding and continuity of care.
* Participate in multidisciplinary team meetings and case discussions.
* Patient Safety and Quality of Care:
* Promote a culture of safety and quality within the practice.
* Ensure that care is delivered in accordance with clinical guidelines and standards.
* Identify and report any adverse events or incidents, contributing to a learning environment.


Person Specification


Qualifications

* Registered healthcare professional - e.g., nurse, paramedic, pharmacist - with current registration with the relevant professional body.
* Masters degree in Advanced Clinical Practice or equivalent qualification.
* Significant clinical experience in a relevant healthcare setting.
* Independent prescribing qualification (or willingness to obtain).
* Strong clinical assessment, diagnostic, and decision-making skills.
* Proficiency in using electronic health records and other digital tools to enhance patient care and communication.
* Excellent communication skills, with the ability to engage effectively with patients, families, and multidisciplinary teams.
* Strong interpersonal skills, demonstrating empathy, patience, and cultural competence.
* Experience in leading clinical teams or projects, with the ability to inspire and motivate colleagues.
* Skills in managing change and improving clinical services or pathways.
* Involvement in clinical research or quality improvement initiatives, with a focus on evidence-based practice.
