Your Responsibilities:
Undertake person-centred assessments to identify the needs and goals of individuals with learning disabilities and autism.
Develop, implement, and review care and support plans to promote independence and community integration.
Work collaboratively with service users, families, carers, and healthcare professionals to ensure joined-up care.
Provide advocacy and emotional support, empowering individuals to make informed decisions.
Maintain up-to-date records, adhering to legal and policy standards and contributing to service improvement.
