Overview
We are looking for an enthusiastic and capable nurse with relevant clinical and leadership experience to manage our Community Adult Complex Care Team in Aberdeen City, whilst working closely with multi-professional colleagues and families to improve the pathway for young adults transitioning from children’s services. Applications from NMC registered nurses for Adults, Children and Young People, and Learning Disability will be considered.
Responsibilities
* Provide professional leadership and line management to a team of nurses and healthcare support workers who directly care for individuals with life-limiting physical conditions, which may be combined with a learning disability.
* Balance a defined clinical commitment with management and leadership responsibilities.
* Offer expert advice and support for primary and secondary healthcare teams, families, guardians, social work and social care providers to understand and meet the needs of this group.
* Support complex transitions to adult services and ongoing care.
* Be supported by a Nurse Manager, Deputy Chief Nurse and team leader colleagues across Community Nursing services.
* Contribute to delivering a 24/7 service with flexibility to meet service demands outside standard hours.
