The main purpose of this post is to provide a versatile and responsive service to ensure that opportunities for patients' reablement are optimised within the community setting. The Reablement workers within each locality will form part of an Integrated Care Pathway for older patients, involving a range of services to prevent hospital admission and support discharge from hospital. The post holder will be expected to work with patients in their usual place of care, which may include residential homes and nursing homes. Flexibility in working patterns is required to meet the needs of the service, with a rostered schedule providing an 8-8, 7-day-a-week service.
Key Responsibilities:
1. Work as part of an interdisciplinary team to facilitate holistic reablement for patients.
2. Work under clinical supervision of the locality team, including ward and community staff.
3. Provide an 8-8, 7-day service for patient reablement.
4. Communicate routine information tactfully and persuasively.
5. Persuade patients to cooperate with their reablement programmes, providing reassurance as needed.
6. Begin working towards the Rehabilitation Support Worker and Healthcare Assistant (HCA) qualifications.
7. Complete the Ageing Well competencies training programme.
8. Participate in clinical and peer supervision for ongoing learning.
9. Work alongside clinical facilitators to achieve venepuncture competency, as required.
10. Monitor personal progress, strive to improve practices and performance.
11. Assist patients in achieving reablement goals through activities that promote independence, including support with personal care and the use of aids/equipment, encouraging self-care.
