Overview
We are seeking to recruit motivated practitioners with initiative and drive who are keen to contribute to the further development of our integrated children's health service. The Children's Health Service 0-19, alongside others, supports the delivery of the Healthy Child Programme across Lincolnshire for Children, Young People and their families (aged 0-19 years) and up to the age of 25 years for Young People with Special Educational Needs and/or disabilities (SEND). The team is based across 4 teams in Lincolnshire, ensuring that children, young people and families can access timely expert advice and intervention on specific health issues when needed. The CYPN team works with Children’s Services (Children’s Centres, Early Help, Social Care and Family Hubs) and other agencies to improve outcomes and reduce health inequalities for children, young people and their families in our care.

Responsibilities
* Deliver and support public health messages, including awareness sessions to schools on topics such as anaphylaxis and asthma.
* Work with external partner agencies (e.g., GPs, schools and other health agencies) to support children with complex health needs to meet their full potential, including work around the Educational Health Care Plan and enabling children with medical needs to access education.
* Collaborate with Children's Services and other agencies to improve outcomes and reduce health inequalities for children, young people and their families in our care.
Requirements & Qualifications
* Must be a registered paediatric nurse (or a registered adult/mental health nurse with current paediatric experience) with a current NMC registration.
* A good understanding of children’s safeguarding.
* Experience of supporting a learning environment, including supporting students.
* Excellent organisational and communication skills.
* A good understanding of clinical governance and the CQC (Care Quality Commission) standards for community practice.
* Ability to work in a changing environment and support others in that situation.
* Commitment to working collaboratively and in partnership with early help and social care colleagues to improve outcomes for children and young people.
