Outreach Caseworker
Location: East Kent
Hours: 35 per week
Contract: Fixed term until 31st March 2026 with possibility of an extension
Salary: £30,000 per annum
UK driving licence and access to a vehicle is essential (business mileage will be reimbursed)

Are you a highly experienced HIU Lead in the East Kent area who would love to work on an innovative NHS programme (High Intensity Use (HIU) Programme)?
The aim of the programme is to change the perceptions of people who access health care more than expected. The team help to prevent people from reaching crisis point, empowering them to better self-manage and connect with their surroundings.
What does the role of an Outreach Caseworker involve?
1. Working in the community to help identify and address unmet social needs that may be exacerbating physical and/or mental health. Identifying what causes high usage of emergency care services.
