As an Intensive Prevention Practitioner, you will work with children aged 10-18 years who are at risk of becoming Looked After by the Local Authority. The team will undertake intensive, targeted, and creative work with families, including one-to-one and family sessions, to strengthen relationships and reduce the likelihood of entry into care.
The purpose of this post is to provide intensive support to young people and their families to overcome barriers impacting their health, social, emotional, and educational needs, aiming to improve outcomes and help them reach their full potential while remaining within their families. You will:
* Provide timely and effective interventions to support young people and their carers to reduce the risk of children becoming looked after.
* Ensure the most appropriate outcomes for families through building strong relationships with young people and their families/carers.
* Enable families to function as effectively as possible.
* Build constructive, supportive, professional relationships with families, keeping the needs and views of children, young people, and their caregivers central.
* Deliver tailored, evidence-based interventions to individuals, families, and groups as required.
* Develop and implement support plans in collaboration with families and other professionals, reviewing regularly.
* Undertake intensive home visiting and support using solution-focused approaches and evidence-based practices.
* Provide information about local services relevant to family needs and support access.
* Participate in a duty rota system, including working evenings and weekends.
* Maintain accurate case records and documentation as per departmental guidance.
What you need to have:
* Educated to Level 5 in a related field.
* Substantial experience working with children and families with complex needs.
* Experience working with young people and families exhibiting emotional and behavioral difficulties.
* Experience with creative and innovative support approaches for vulnerable and hard-to-engage families.
