This opportunity is now available in the Chalfont's and Gerrards Cross area to become a Community Nurse within a newly formed team. As a community nurse, you would be supported to develop into an autonomous practitioner; nursing patients with multiple complex health care needs in their own home.
Main duties of the job
Community nursing is a specialised area of practice. As a Community Staff Nurse, you will work within a locality team of District Nurse Team Leaders, Community Staff Nurses, Therapists, Assistant Practitioners and Healthcare Assistants, to meet the needs of the complex patient in the community and avoid admissions to acute settings. This includes medication administration, end of life care, managing the deteriorating patient, complex and simple wound care including negative pressure wound therapy and compression therapy, venepuncture, utilising advanced assessment skills and clinical nursing skills. We nurse people with a variety of conditions in a holistic manner in their own home and in residential care settings. Community nurses work in close partnership with social care and other primary health care professionals such as GP’s, Community Therapies and specialised services such as Tissue Viability, Diabetes and Heart Failure.

EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
10. Registered Nurse/Awaiting NMC PIN
Desirable criteria
11. Committed to own professional development to enhance practice
12. Post registration education and extended skills e.g. Intravenous drug administration, compression dressings.
13. Evidence of further continued professional development
14. Post qualification experience in relevant speciality
15. Hold / be working towards an NMC approved mentoring qualification
SKILLS, ABILITIES & KNOWLEDGE
Essential criteria
16. To be able to communicate effectively with patients, relatives, carers and other professionals.
17. To be able to care for patients without prejudice.

19. Ability to manage own caseload.

23. Demonstrate experience post or pre-registration placements in relevant clinical areas: Care of Elderly Community Orthopaedics Neurology Respiratory Palliative Care Long term conditions
24. Ability to understand the role of a registered nurse
25. Ability to work as part of a team and on own initiative
26. Awareness of current professional nursing issues
27. Demonstrates understanding of the importance of audit/quality
Desirable criteria
28. Desirable but not essential knowledge of working with a single assessment, planning and the delivery of care.
29. Experience of electronic clinical systems and technology e.g. RIO
