The Community Rehabilitation Team is looking to recruit an experienced Community Support Worker to join our supportive multidisciplinary team. The successful candidate will have significant experience of working with people with chronic relapsing and remitting mental ill health, including experience of working across a number of services, including secondary care and mental health inpatient services.

The team supports service users to transition from inpatient rehabilitation facilities to living in the community, supporting people to improve their quality of life and work towards achieving their goals. A further aim of the team is to reduce the length of stay for service users from acute and rehabilitation wards, incorporating least restrictive practice.

The team has an emphasis on the Recovery Model, working with service users using a strengths based and trauma informed approach to maximise their independence and autonomy, to improve their quality of life.

You will collaborate with a team of experienced practitioners, utilising evidence-based interventions to promote social inclusion and support mental health recovery.

The team is a trust wide service covering all five boroughs of Pennine Care and it is essential that you drive to enable you to travel across the Trust footprint.


Main duties of the job

To work as part of the multidisciplinary team and provide support on both an individual and group basis to individuals experiencing complex psychosis.

To support service users to achieve their full potential through accessing resources within their wider communities and equipping them with the skills and confidence to enable them to manage their lives independently.

The role will involve delivery of therapeutic recovery focused and community activities, and individualised treatment plans under the guidance of practitioners within the team.

To provide support to an allocated group of service users, promote their recovery and help to maintain them in their community environment.To assist qualified practitioners in assessment, planning, implementing and evaluating individual care plans.To ensure that service users engage effectively with the agreed care plan and access appropriate services.Positively promote independent living of service users within the community.Provide support and interventions to service users in their own homes and the wider community based upon their assessed needs.Support service users to become involved in social activities in line with their assessed needs and planned care.Support service users to establish and maintain effective interpersonal relationships.Provide regular and practical support for service users and their carers in managing dignity and independence.

Provide support with activities of daily living.

Job responsibilities

* Good standard of general education including GCSE Maths and English grade C and above or equivalent.
* Level 2 qualification in Healthcare plus training and/or experience to Level 3
* Evidence of recent further development / training
* Experience of working with marginalised people with complex presentations who may have substance use/homeless.
* Experience of working with individuals with severe and enduring mental health needs
* Experience of working with individuals in distress
* Experience of working with service users who present with risk.
* Understanding of mental health issues and how these impact on individuals function
* Good written and verbal communication skills
* Ability to listen effectively
* Ability to implement care plans for individuals with the support of others as appropriate.
* Ability to create innovative solutions to help empower service users.
* Ability to work unsupervised in a range of settings
* Ability to form positive therapeutic relationships with service users and carers
* Willingness to make a positive contribution to improving the quality of life for people with mental health problems.
* Ability to provide practical support with daily living activities.
* Basic IT Skills
* A current driving license with business insurance and access to a car during the working day is essential
* Experience working within rehabilitation services
* Experience working in the NHS.
* Experience working in a community setting
* Awareness of local services within the Pennine Care footprint.
* Knowledge of benefits/employment systems.
* An understanding of the mental health system.

* An understanding of the mental health system.
