Job Description - Support Coordinator (25868)

Support Coordinator

Location: West Yorkshire Supported Accommodations and Community services

Type: Permanent, Full time (37.5 hpw). Applications for part-time will be considered.

Salary: £13.21 per hour (£25,838 per annum) [pro rata] plus benefits including Health Cash Plan.

Join a team that cares and empowers you to make a difference in our customers' lives. As a Lead Support Worker, you'll support customers with complex needs in their accommodations and community. Our Central Resource Team ensures continuous support for our accommodations, covering staff leave, sickness, or recruitment.


Typical Day as a Support Coordinator

* Develop support plans with customers and coordinate a team of Support Workers.
* Assist customers with complex needs such as homelessness, substance misuse, mental health, and social isolation.
* Support independent living skills, daily tasks, budgeting, health, wellbeing, and activities.
* Conduct risk assessments, support planning, goal setting, safeguarding, and reviews. Assist with tenancy setup and termination.
* Engage in conversations, collaboration, and building rapport with customers and colleagues.


You Bring

* Passion for supporting customers to live their best lives with attention to detail.
* Experience in creating person-centered support plans and guiding colleagues.
* Experience in coordinating and assessing customer referrals.
* Ability to work independently, stay calm under pressure, and be resilient.
* Valid vehicle insured for business purposes; mileage paid.

Join Yvonne, part of the Central Resource Team since 2014, supported by Client Service Manager Laura, a former Support Coordinator.


Job Details

* 37.5 hours/week, including evenings and weekends, on a rota basis. Part-time considered.
* Travel across West Yorkshire, including Huddersfield, Wakefield, and Leeds.
* Flexible start and finish times to meet customer needs.
* Regular training and team meetings, with occasional lone working.
* Use technology for support plans, online learning, and collaboration.
* Enhanced DBS check required; paid for by us.
