An exciting opportunity has arisen for the Adult Community Nursing Service. We are currently seeking Band 5 Community Nurses to work across the Hounslow areas, based at the Bedfont Clinic, Imperial Road. The service operates from 07:00 to 19:00, seven days a week, including alternate weekends, with shifts of 07:00-15:00, 08:30-16:30, and 11:00-19:00.
Collaborating with our integrated rapid response teams, community matrons, and social service colleagues is essential to ensure seamless care for our patient group.
The role involves providing high-quality, skilled nursing care and supporting the Community Nursing Team Manager in planning, implementing, and evaluating holistic patient, family, and carer needs.
You will deliver high-quality nursing care to housebound patients with complex needs, enabling them to remain in their own homes. Working closely with specialist nurses, allied health, and social care professionals helps meet patient needs without unnecessary A&E visits or hospital admissions.
This position involves working independently within a designated geographical area, conducting assessments and reviews, planning, implementing, monitoring, and evaluating individual care programs. Flexibility to support the corporate approach to patient care is expected.
